As a Licensed Social Worker, you'll work with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population spanning the entire lifecycle, and all member contact will be done telephonically or via electronic exchange. **Responsibilities** * Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Work with a population spanning the entire lifecycle * Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs * Empower members to make positive lifestyle and medical choices * Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Maintain accurate and up-to-date records of member interactions and progress **Requirements** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence * 2+ years of experience in medical social work or case management * Masters Degree in Social Work **Preferred Qualifications** * Experience working with diverse populations, including children, adults, and older adults * Knowledge of community resources and services for medical, financial, and behavioral health * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Strong problem-solving and critical thinking skills **Benefits** * Competitive pay range: $46,597.20 - $101,228.40 * Bonus, commission, or short-term incentive program * Comprehensive benefits package, including medical, dental, and vision insurance * 401(k) retirement savings plan * Employee Stock Purchase Plan * Fully-paid term life insurance plan * Short-term and long-term disability benefits * Paid Time Off (PTO) or vacation pay * Paid holidays throughout the calendar year * CVS store discount * Discount programs with participating partners **Work Environment** * Remote work from home role * Must be able to work anywhere in the U.S. * Working hours: Monday-Friday, 8am-4:30pm Pacific Standard Time * No weekends or holidays required **How to Apply** If you're passionate about delivering enhanced human-centric health care and making a difference in the lives of others, we encourage you to apply for this exciting opportunity.
