**Job Summary**
We're seeking an experienced Licensed Social Worker to join our remote team. As a Licensed Social Worker, you'll work with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population spanning the entire lifecycle, and all member contact will be done telephonically or via electronic exchange.
**Responsibilities**
* Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Work with a population spanning the entire lifecycle
* Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
* Empower members to make positive lifestyle and medical choices
* Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Maintain accurate and up-to-date records of member interactions and progress
**Requirements**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
* 2+ years of experience in medical social work or case management
* Masters Degree in Social Work
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and older adults
* Knowledge of community resources and services for medical, financial, and behavioral health
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
