**Job Summary**
We're seeking an experienced remote social worker to join our team of dedicated professionals. As a medical case manager and community resource coordinator, you'll work collaboratively with registered nurses and other clinical staff to assess, plan, and advocate for the needs of our patients. Your expertise will be instrumental in connecting patients with essential resources, empowering them to make positive lifestyle and medical choices, and promoting their overall well-being.
**Responsibilities**
* Collaborate with registered nurses and other clinical staff to assess, plan, and advocate for the needs of patients
* Research and connect patients with available options and resources for financial, transportation, safety, and housing needs
* Empower patients to make informed decisions about their health and connect them with essential resources
* Work as a team with other clinical staff to assist patients with community resources for medical, financial, and behavioral health
* Utilize a collaborative process of assessment, planning, and advocacy to meet patients' benefit plan and/or health needs
* Conduct telephonic or electronic exchanges with patients to assess their needs and provide support
* Develop and implement personalized plans to address patients' needs and promote their overall well-being
**Qualifications**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
* 2+ years of experience in medical social work or case management
* Masters degree in social work or a related field

**Compensation and Perks**
We offer a competitive salary range of $46,597.20 - $101,228.40, depending on experience and qualifications. In addition to your base salary, you'll also be eligible for a CVS Health bonus, commission, or short-term incentive program. Our comprehensive benefits package includes:
