RN Consultant – CHAP & ACHC Accreditation
Home Health & Hospice | Remote with Limited Travel

Travel: 1–2 days approximately once per month or every other month

About the Opportunity
We are seeking an experienced Registered Nurse with proven CHAP and/or ACHC accreditation expertise in home health and hospice to join our national consulting team.
This is a primarily remote consulting role focused on helping agencies achieve and maintain successful accreditation outcomes through clinical compliance guidance, mock surveys, documentation review, and survey readiness support.
The ideal candidate brings hands-on accreditation experience, strong regulatory knowledge, and the ability to confidently guide leadership teams through the accreditation process from preparation to successful survey.
Key Responsibilities
• Lead CHAP and ACHC accreditation readiness for home health and hospice clients nationwide
• Conduct mock surveys, chart audits, and clinical compliance reviews
• Interpret Medicare Conditions of Participation and translate requirements into practical action plans
• Provide clinical policy guidance and corrective action recommendations
• Prepare agencies for initial accreditation, re-accreditation, and post-survey follow-up
• Deliver virtual training and consultation to administrators, DONs, and clinical teams
• Collaborate with internal licensing, compliance, and consulting staff
• Travel periodically for short on-site readiness or survey support visits
Qualifications
• Active Registered Nurse (RN) license in good standing
• Direct experience with CHAP and/or ACHC accreditation in home health and/or hospice
• Strong knowledge of Medicare CoPs, survey process, QAPI, and clinical documentation standards
• Experience performing mock surveys, chart reviews, or compliance audits
• Ability to work independently in a fully remote consulting environment
• Excellent communication, organization, and training skills
• Willingness to complete occasional overnight travel

Job Types: Full-time, Permanent
Pay: $75,000.00 - $80,000.00 per year
