Position Summary
To provide direct services as a part of a multidisciplinary team the master’s prepared clinician will provide direct care including individual (particularly children/adolescents), couple, family and group therapeutic services to patients, including assessments, treatment plans, crisis intervention and discharge planning.
Essential Duties & Responsibilities
Direct Patient Contact:
- Provide individual and family therapy to children and their families
- Participates in direct care to assess needs and coordinate services
- Maintains daily/weekly scheduled appointments in electronic records systems
- Provide therapeutic services to patients and their families
- Work in collaboration with multidisciplinary team members including external partners
- Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner
- Participate in our high risk program
- Demonstrates cultural competency and commitment to patients including LGBTQ+ community
- Maintain case load dependent on need
- Participates in clinical supervision with LCSW
- Provides on-call support on a annual rotational basis if required by program
- Provide crisis management, support and triage
Minimum Qualifications
- Master’s degree in social work, counseling, marriage and family therapy
- Possession of valid clinical license in Connecticut (LMSW, PC-A, MFT-A)
- Valid driver's license a must for in home services
- Comfort with using DSM 5 and understanding diagnoses
- Must have excellent writing and communication skills, be highly organized
- Must be able to work as a part of a team
Preferred Qualifications
- LCSW, LPC, LMFT preferred
- Previous experience providing clinical care is desirable
- Experience/knowledge of substance use disorders
- Experience/knowledge in working with LGBTQ+
- Previous home visiting experience
