Make a Difference in Your Community as a Community Connector We are seeking a compassionate and results-driven Community Connector to join our team in Dayton, OH. As a Community Connector, you will play a vital role in connecting our members with the resources and support they need to manage their healthcare. This is a unique opportunity to make a positive impact in your community and help individuals navigate the healthcare system. About the Role: In this role, you will serve as a community-based member advocate and resource, using your knowledge of the community and available resources to engage and assist vulnerable members in managing their healthcare needs. You will collaborate with our Healthcare Services team to provide non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices. Responsibilities: Empower members by helping them navigate and maximize their health plan benefits Assist members in accessing social services such as community-based resources for housing, food, employment, and more Provide outreach to locate and support disconnected members with special needs Conduct research to locate members who have been unable to be contacted Participate in ongoing or project-based activities that may require extensive member outreach Guide members to maintain Medicaid eligibility and access other financial resources as needed Requirements: High school diploma or equivalent required Minimum 1 year of experience working with underserved or special needs populations Valid driver's license with a good driving record and reliable transportation Active and unrestricted Community Health Worker (CHW) Certification (required for Ohio, Florida, and California) Preferred Qualifications: Associate's degree in a healthcare-related field LPN (Licensed Practical Nurse) certification Bilingual skills based on community need Familiarity with healthcare systems and community-specific culture What We Offer: Competitive salary range: $16.40 - $31.97 per hour Comprehensive benefits package Opportunities for professional growth and development A dynamic and supportive work environment How to Apply: If you are passionate about making a difference in your community and have a strong desire to help others, we encourage you to apply for this rewarding role.
