This is a dynamic, positive, one-of-a-kind environment that actively promotes the health and well-being of team members and persons served! This position provides care coordination to people with disabilities based on assessments completed.

Duties/responsibilities include but are not limited to:

– Conduct Assessments (remotely or in person based on member preference)

– Ensure member receives necessary care & services

– Review current health care needs, identify goals and develop a care plan

– Coordinate with members’ teams to ensure health care quality is met

– Advocate for members

– Manage Waiver services

– Manage client files and keep detailed notes

Education:

– RN or LSW (Preferred)

Or

– Bachelor?s degree in social work, psychology, sociology or related field and at least one year of experience in health care or human services field (required)

Qualifications:

– Pass background study

– Maintain professional boundaries & confidentiality

– Professional verbal and written communication skills

– Accept & adapt to changes

– Creative thinker & professionally solve problems effectively

– Take initiative, work independently with minimal supervision

– Computer skills

– Be dependable

– Organized and attentive to detail

– Valid driver?s license and a reliable, insured vehicle with a clean driving record

Wages:

– $45,000/annually – $55,000 annually, based on experience

Frequently Asked Questions

Hours- Schedule is determined by staff and consumers. Traditional hours are 8-4:30 but are flexible. Staff can create their own schedule.

Mileage reimbursement- Mileage is reimbursed at .58 cents per mile

Working remotely- Yes, position is remote with occasional travel.

Business cell phone and computer- Yes

Evaluations – Evaluations occur every September for the year. All employees have monthly 1:1s with their supervisor to consistently evaluate performance. In Sept the evaluation is a monthly 1:1 but encompassing the whole year.

Advancement or Promotion within the company- There is always room for advancement and promotion and is encouraged that our employees move in that direction if that is their goal.

Bonuses or Incentives- We have discretionary bonuses that can occur based on performance
