Health Choice Network is seeking a skilled Healthy Planet Application Analyst to support the configuration, optimization, and ongoing support of population health functionality within our Epic environment. In this role, you will collaborate with both operational and technical teams to enhance population health workflows, improve data-driven care initiatives, and ensure Epic solutions align with organizational and community health goals.

Key Responsibilities
- Design, build, test, and maintain Epic Healthy Planet workflows and system configurations
- Support population health initiatives by optimizing registries, reporting tools, and care management workflows
- Identify, troubleshoot, and resolve technical and workflow issues impacting system performance
- Participate in Epic upgrades, enhancements, and implementation initiatives
- Serve as a subject matter expert and primary support resource for Healthy Planet users
- Collaborate with operational leaders and technical teams to translate business needs into effective system solutions
- Support training initiatives and end-user education to ensure successful adoption
- Ensure system workflows align with population health strategies and compliance standards
Required Qualifications
- Epic Healthy Planet Certification and/or Epic Ambulatory Certification (required)
- 3+ years of experience supporting Epic applications or healthcare IT systems
- Experience with Epic build, workflow design, testing, and troubleshooting
- Knowledge of population health, community health, and public health initiatives
- Strong verbal and written communication skills
- Ability to translate technical requirements into solutions for non-technical users
Preferred Qualifications
- Experience working with population health reporting, registries, and analytics tools
- Experience supporting Epic upgrades and cross-module integrations
- Background supporting healthcare operations, clinical systems, or care management programs
